How long do opiates stay in your system?
That range is typical rather than a guarantee: how much was taken, how often, body weight and hydration all move it.
The screening cutoff above is the level at or above which the on-site cassette reads positive. Below it the panel reads negative even when a trace is present, which is why the cutoff matters as much as the number of days.
Can a prescription cause a positive on the Opiates panel?
Codeine or morphine prescriptions, and even poppy seeds, can cause a positive. Lab confirmation reports measured levels, and every laboratory report goes to a Medical Review Officer (MRO), a physician who reviews laboratory-confirmed results and accounts for valid prescriptions before the report is released.
What does a positive on the Opiates drug test panel mean?
The on-site read is a drug screen. It reports positive or negative against the screening cutoff, not a measured level. Laboratory confirmation is what produces a number: a CAP-accredited laboratory quantifies the specimen by LC-MS/MS and reports measured levels, in about 72 hours. Every laboratory report goes to a Medical Review Officer (MRO), a physician who reviews laboratory-confirmed results and accounts for valid prescriptions before the report is released. Confirmation can be added when you book, or from the app within 2 hours of collection, while the specimen is still viable.
Codeine, morphine, heroin: what the opiates panel reads
The MOR panel is built around morphine and the natural opiates related to it. Codeine, found in some prescription cough syrups and in pain tablets such as Tylenol with Codeine, reads on it, and so does morphine itself, prescribed for pain. So does heroin, which the body turns into morphine. Cups and laboratory reports print this panel as MOR, MOP or OPI, and the number beside the code matters as much as the letters: a Testr screen reads it at 300 ng/mL, while a cup printed OPI2000 is the same panel set at 2,000 ng/mL, so one specimen can read positive on one cup and negative on the other.
Opioids that need a panel of their own
Several opioids people expect an opiates test to catch are different molecules, and the cassette screens them separately: oxycodone on the OXY panel, fentanyl on FEN, methadone on MTD and buprenorphine on BUP. Hydrocodone and hydromorphone have no panel of their own on the cassette, and Testr does not claim the MOR panel reads them at its cutoff. Tramadol and kratom are not on the rapid screen at all; both are laboratory questions. So is tapentadol (Nucynta), another prescription pain medicine, which the laboratory menu lists at 50 ng/mL.
Poppy seeds, heroin and the laboratory menu
Poppy seeds carry traces of morphine and codeine, so a MOR line on the cassette says the class is present, not where it came from. Laboratory confirmation takes the class apart. Its menu lists codeine, hydrocodone, hydromorphone, morphine and oxymorphone, at 50 ng/mL each, and lists 6-acetylmorphine, the breakdown product specific to heroin, on its own at 10 ng/mL. A measured level for each compound, with or without 6-acetylmorphine beside it, is what a reviewing physician reads.
